FoneYam will be insured against your Death, Disability, Loss of Income or should your device be Lost or Stolen.  

What do I need to submit a claim?

Lost or Stolen Device

Report your device as lost or stolen at the nearest police station and receive your South African Police Service (“SAPS”) case number.

✅ Complete an affidavit that includes: Your full name and surname, ID or Passport number, Contract Cellphone number, IMEI number of the device, ITC number and SAPS case or OB number and a brief description and date of the incident.

✅ Certified copy of your ID or Passport.

Death

✅ Certified copy of ID or Passport of Deceased.

✅ Death Certificate of Deceased.

Disability

Abacus, the insurer, will be in touch with further instructions once a claim has been logged. Prepare the following documentation in the meantime:

✅ Affidavit of lost income

✅ Medical report on disability

✅ Employer termination letter

✅ UI19 confirming illness termination (Code 10)

Loss of Income

Abacus, the insurer, will be in touch with further instructions once a claim has been logged. Prepare the following documentation in the meantime:

✅ Affidavit of lost income

✅  3 months bank statement

✅ UI19 form from Employer

✅ Retrenchment letter from the Employer

What happens once the claim is approved?

When the Death, Disability or Loss of Income claim is approved, you will no longer have to continue paying your contract.Once the Lost or Stolen Device claim is approved:

  • If your contract was concluded on or after 4 November 2024 you will receive a replacement device**, and you will continue with your original contract.
  • If you accepted the terms and conditions on or after 20 April 2026, the contract will terminate on the date we inform you that our claim has been approved and you will no longer have to continue paying your contract.

**If all outstanding payments have been paid.
